Mental Health Self-screening Tool

Home » Mental Health Self-screening Tool

The self-screening tool uses Depression Anxiety Stress Scale 21-items (DASS 21), which is a psychological test that is often used for initial assessment of the anxiety-depression-stress level, which was launched by the experts of the University of New South Wales in 1995.

Quickly assess your condition for advice and support when needed. You can do this test when necessary and redo it anytime.

If you are willing to receive support from the factory’s health workers/personnel department based on your assessment – please select the factory name and fill in the personal information below. The information you provide and the evaluation results will be completely confidential and only the health workers/personnel department in charge of support will have access to them.

If you are not willing to provide personal information – please select the factory name and start the self-assessment immediately.

Please read each statement and circle a number 0, 1, 2 or 3 which indicates how much the statement applied to you over the past week. There are no right or wrong answers. Do not spend too much time on any statement.
